IF YOU RECEIVE A DECLINATION ON A CLAIM FORWARD IT TO THE LOCAL CHAIRMAN VIA TSS 201. PLEASE FILL OUT AN R-100 AND INCLUDE ANY RELEVANT INFORMATION, PLACE IT INTO THE BLE CLAIMS BOX AT EOLA OR CICERO THANKS!

- R-100 Form filled out in deatil

- Switching claims need copy of switch list
